Our Mission
At BCHN, we are committed to improving the health and well-being of our Bronx community. That means providing access to the highest quality health and wellness care for people of all races, genders, sexual orientations, income levels, and life circumstances.
Through our health care centers, our programs to encourage healthy lifestyles and prevent and manage chronic illnesses, and our outreach to families, teens, adults, and elders, we are creating a more equitable and community-centric model of care for all.
History
Just a year later, we received our first federally qualified health center grant, partnering with Montefiore Health System and Promesa Systems, and soon expanded into HIV/AIDS care at ten Montefiore health centers, as well as opening the first of nine school-based health centers. Our network of centers has grown to 18 locations, serving approximately 100,000 patients annually.
In the early 2000’s, oral health and mental health programs were launched, along with our successful community health worker (CHW) program to reach people where they live. Our CHW team has blossomed year by year, and is among the most effective ways to connect our patients to care and to the support services they need. Our focus on parents and children, starting with prenatal care, began with our Healthy Family, Healthy Baby program. Later, our Healthy Start program, partnering with the Albert Einstein College of Medicine, gave families the information and tools they need to succeed as a family.
From 2010 to 2018, BCHN launched programs for smoking cessation, and prevention or management of diabetes, asthma, and high blood pressure. We have responded to the opioid crisis through workshops on the use of naloxone to reverse an opioid overdose.
The COVID-19 pandemic saw BCHN in full response mode, providing nearly 7,000 antibody and diagnostics tests, and expanding safe and easy access to care through ramped up telehealth services. And we put our Outreach Van to work, partnering with Project Bravo food pantry to bring nourishment to those facing food insecurity.
