Bronx Community Health Network (BCHN) is a not-for-profit organization and Federally Funded Health Center that sponsors 21 community and school-based health centers in the Bronx.
BCHN health centers offer high-quality medical care and related health services for free or at low cost for the entire family, at all stages of life.
BCHN director of programs Tashi Chodon and quality improvement coordinator Winsome Ramsey presented a poster at this year's National Association of Community Health Centers (NACHC) Community Health Institute (CHI) in late August. The poster highlighted BCHN's work "Addressing Social Determinants of Health Through Integrating Community Health Workers into Health Center PCMH Care Teams."
BCHN Board of Directors member Marjorie A. Cadogan and community health worker supervisor Patrizia Bernard appeared on Bronxnet's OPEN on December 17 to discuss open enrollment for 2019 health insurance.
